Enalapril is a prodrug belonging to the angiotensin-converting enzyme (ACE) inhibitor drug class that works on the renin-angiotensin-aldosterone system, which is responsible for the regulation of blood pressure and fluid and electrolyte homeostasis. Enalapril is an orally-active and long-acting non-sulphydryl antihypertensive agent that suppresses the renin-angiotensin-aldosterone system to lower blood pressure. It was developed from a targeted research programmed using molecular modelling.Being a prodrug, enalapril is rapidly bio-transformed into its active metabolite, enalaprilat, which is responsible for the pharmacological actions of enalapril. The active metabolite of enalapril competitively inhibits the ACE to hinder the production of angiotensin II, a key component of the renin-angiotensin-aldosterone system that promotes vasoconstriction and renal reabsorption of sodium ions in the kidneys. Ultimately, enalaprilat works to reduce blood pressure and blood fluid volume.

Indications :

Enalapril is used to treat high blood pressure. Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. It is also used to treat heart failure and to help prevent people with a certain heart problem (left ventricular dysfunction) from developing heart failure.

Contraindications :

  • Hypersensitivity to enalapril, to any of the excipients or any other ACE inhibitor
  • History of angioedema associated with previous ACE-inhibitor therapy
  • Hereditary or idiopathic angioedema

 

Side effects :

Dizziness, light-headedness, or weakness may occur as your body adjusts to the medication.

Pharmacodynamics :

Enalapril maleate is the maleate salt of enalapril, a derivative of two amino acids; L-alanine and L-proline. Angiotensin-converting enzyme (ACE) is a peptidyl dipeptidase which catalyses the conversion of angiotensin I to the pressor substance angiotensin II. After absorption, enalapril is hydrolysed to enalaprilat, which inhibits ACE. Inhibition of ACE results in decreased plasma angiotensin II, which leads to increased plasma renin activity and decreased aldosterone secretion.ACE is identical to kinase II. Thus Enalapril Maleate Tablets may also block the degradation of bradykinin, a potent vasodepressor peptide.

Mechanism of action :

While the mechanism through which Enalapril Maleate Tablets lowers blood pressure is believed to be primarily suppression of the renin-angiotensin-aldosterone system. Enalapril Maleate Tablets are antihypertensive even in patients with low-renin hypertension.

Storage :

Enalapril maleate alone is very stable in temperature under dry and humid conditions, while the tablets are stable if protected from these same conditions.
